Komaru begins as a somewhat hypersensitive and cowardly person who often wavers between hope and despair. She doesn't want to have strong hope, because she believes it will be lost eventually, leading to worse despair. She has low self-esteem, considering herself boring, ordinary, and weak. She also shows some childish traits, such as being naive, reliant, and easily excitable. She also has an 'unusual' kind of pride, as she gets really worked up from the puzzles of the Monokuma Kids and brags towards the kids when she solves them. She is also very gullible and rather innocent as well, as doesn't seem to have much interest in romance nor sexual things. It's been stated that in the past some creepy guys have taken interest in her, supposedly because of her gullibility, and it's been shown that she isn't very good at noticing their intentions.

While a kind person, Komaru sometimes has a hard time relating to certain people and subjects, because she herself has lived a very ordinary life. However, she herself has a few very unusual traits as well, including her strange favorite foods and her apparent ability to sense ghosts, although she doesn't seem to think these traits are anything unusual. Despite sometimes giving up too easily, there are other times which Komaru shows how stubborn and determined she can be. She has a weakness in complicated subjects such as basic computer science, but she does have moments where she is intuitive.

A group of 15 elite high school students are gathered at a very special, high class high school. To graduate from this high school essentially means you'll succeed in life, but graduating is very difficult. The school is presided over by a bear called Monokuma, and he explains to them that their graduation hinges around committing a murder. The only way to graduate is to kill one of your classmates and get away with it. If the other classmates discover the identity of the killer, the killer is the only one executed. However, if they fail to catch the killer, only the killer graduates and the others are annihilated. Which of the 15 will survive the bloodbath to come?

Makoto Naegi, the Ultimate Hope who defeated the Ultimate Despair, Junko Enoshima, is under suspicion of treason against the Future Foundation for harboring the Remnants of Despair who carried on her will. As top members of the Future Foundation, including Makoto's former classmates Kyoko Kirigiri, Aoi Asahina, and Yasuhiro Hagakure, are brought together to decide on Makoto's punishment, they are all imprisoned by Monokuma, who forces everyone to participate in a final killing game. They must try to find the traitor among them who will kill them in their sleep, while also trying to avoid being mortally poisoned by personalised forbidden actions. Thrust into a game where no one can trust each other, Makoto's ideals of hope are pushed to their very limits.

Their leader, Monaca, declares that the city will be the site of a “children's paradise,” and to accomplish this, all the adults will be exterminated. Komaru teams up with Toko Fukawa to try to survive the rampaging Monokumas, escape the crafty Monokuma Kids, and uncover the secrets of the city.
